An event every 2 months that begins at 12:15 pm on day Second of the month, repeating indefinitely
Workgroup meeting for all CM/CCs working with HVPA practices to ensure all are updated with program changes and on track to hitting target measures. During the meeting, we will discuss current target rates of incentivized care management programs, CM/CC workflow suggestions, and PDCM billing questions and offer this time to collaborate amongst each other for any challenges you may be facing in your own practice as the care manager/care coordinator.
